Introduction
To
be considered for surgery you must go through a screening
process. Again, this thorough process is to ensure that bariatric
surgery is the best solution for your health.
The program progression includes:
- Fill out and return
- Patient
History Questionnaire
- Include a copy of your insurance card - Remember
to copy both the front and back
- If your insurance policy requires it, a copy of your
referral authorization
- You should download, print,
and review the Patient
Guidebook
- You should download and
print the psychologist information sheet
- The questionnaire is reviewed to determine eligibility
- The patient is contacted by letter with a consultation
appointment date/time, or an explanation describing why
they do not meet criteria and suggested alternatives
- The consultation appointment with surgeon, and
bariatric nurse
- At your consultation appointment you
will
- Be given a pre-operative checklist of items to
complete prior to surgery
- Be able to register for the mandatory pre-operative
education classes
- Once a surgery date is scheduled, you
have pre-operative appointments
- Once is with the anesthesiology department
- The second is with the surgeon
- Surgery is performed at UCLA Medical
Center, Westwood
- Post-op follow up appointment with
the surgeon
